Your First Visit

When you visit Lung & Chest for the first time, there are some things that you can do to make the most of your appointment:

  • You will be asked to fill out a form about your medical history. (You may download and print out the medical history form HERE and bring it with you to save time.)
  • Please bring any chest X-rays, CT Scan, or lung scans you have had during the past six months. [We need the actual x-rays.] If you have the report from your tests you may also bring it.
  • Please bring all of your medications including inhalers and patches to your visit. This allows us to confirm what you are actually taking and reduces the chance for medication errors.
  • Please bring a picture ID to your visit. It is also a good idea to bring a written list of questions you may have so that you don't forget to ask them while you are here.
  • Please bring your insurance cards. If your insurance company requires a referral, please bring this at the time of your appointment.
  • We also ask that you do not wear perfumed or scented products to the office as these products bother many people who have breathing problems.

When you enter our office you will be greeted by one of the front office staff who will ask you if you have any updated information since your last visit, so that your patient file and insurance claims are accurate.

Whenever you call the office, you will speak to one of these ladies. When calling, please be prepared to answer some preliminary questions so that they can connect you with the person that can best answer your questions.
